Why Do Aligners Move Teeth?

Some people may wonder whether an aligner can move teeth. In fact, both Invisalign and traditional wire braces move teeth by making use of a biological mechanism called “bone remodeling.”

The root of a tooth is supported by a bone called the alveolar bone, with a thin, cushion-like membrane called the periodontal ligament in between.

When an aligner applies sustained force to a tooth, the periodontal ligament on the side the force is pushing toward compresses, while the ligament on the opposite side is pulled and stretches.

On the compressed side (the direction of movement): cells that dissolve bone (osteoclasts) become active, creating space for the tooth to move into.

On the stretched side (the original position): cells that build bone (osteoblasts) become active, forming new bone to support the tooth.

[Differences From Wire Braces, and Safety]

Wire braces tend to pull teeth with strong force all at once, but each Invisalign aligner moves a tooth only a tiny distance, about 0.25 mm, at a time. This approach of applying “sustained, light force” reduces pain and also lowers the risk of root resorption, in which the tooth root shortens.   • You need to wear the aligners more than 22 hours a day: following the wear schedule is essential. If you don’t wear them long enough, the teeth won’t move as planned, which can extend the treatment period. We make a point of emphasizing this before treatment begins.
  • You must take them out to eat or drink: for anything other than water, you need to remove the aligners to prevent staining or cavities. If you’re worried about forgetting to put them back in, it’s better for tooth movement to just leave them in, but if food gets trapped inside, you do need to take them out and clean them.
  • Some cases cannot be treated: if a skeletal problem is too severe, for example, Invisalign alone can be difficult to use (in that case, we will also propose other treatment options). This depends on what you want to achieve with orthodontic treatment, so you and your dentist need to agree on your goal.

First Visit Counseling (Free)

We ask about your current concerns and your ideal smile. We take the time to explain an outline of treatment, roughly how long it will take, and the cost.

Detailed Examination & iTero Scan

We take X-rays, intraoral photos, and a 3D scan using iTero. If you have any cavities or periodontal disease, we treat them here before orthodontic treatment begins.

3D Simulation (ClinCheck) & Explanation of the Plan

Based on the data we’ve captured, we check a simulation video of how your teeth will move through to the end of treatment, together with you. We then present the final treatment time, plan and cost, and order the aligners only once you’re satisfied.

Treatment Begins (Receiving Your Aligners)

We hand over your completed aligners and go over how to put them in and take them out, how to care for them, and what to watch for. From here, you’ll start wearing them more than 20 hours a day. Attachments are added to your teeth if needed.

Regular Visits (Once Every 1–2 Months)

You’ll have regular visits so we can check whether your teeth are moving as simulated and whether there are any issues in your mouth. We hand over new aligners as treatment progresses.

Retention Period (Wearing a Retainer)

Once your teeth have reached their ideal position, the orthodontic phase is complete. However, teeth that have just moved still have a force pulling them back toward their original position. To keep your beautiful smile for life, you’ll need to follow your retainer schedule.

Common Questions

How long does treatment take?

This varies by case, but it generally takes about 1 to 3 years. For mild partial orthodontics, some cases finish in a few months to about half a year. We offer 2-⁠month, 6-⁠month and 9-⁠month plans as a guide, and will give you an approximate treatment time at your first counseling session.

Does it hurt?

Because each Invisalign aligner moves teeth only a tiny distance (about 0.25 mm) at a time, pain is reduced compared with wire braces. For 2 to 3 days right after switching to a new aligner, you may feel a pushing sensation or mild pain, but most people get used to it within a few days.

How Many Hours a Day Should I Wear It?

Can I Drink Anything While Wearing It?

If you have a cavity or periodontal disease, it needs to be properly treated before orthodontic treatment begins, because if treatment changes the shape of your tooth, the aligner will no longer fit. We also provide general dentistry, so we can support you with everything from cavity treatment to same-day wisdom tooth extraction (depending on the case) before orthodontics starts.

Will It Make It Hard to Talk?

Right after putting it in, you may have minor difficulty pronouncing some sounds, which tends to improve within a few days. It causes less discomfort than wire braces and has almost no effect on daily life.

Is This Covered by Insurance?
